NEED HELP @work?

One of the ways in which you can contact the QPSU to request help with a workplace matter is by using the online form we have created for this purpose. The first part of the form is below.

The form helps you provide us with relevant employment information about you, your preferred contact details, the industrial issue that is the reason for your enquiry and the outcome you are hoping for. To better ensure your confidentiality, the information from the form is processed via our web server (rather than being sent via email).

Before completing the form you must confirm that you have read about the Membership Services Centre, the services we provide and our policies. If you have not done so already, please take a moment to read this policy now.

The form includes a provision for you to affix attachments (eg a Word or PDF document). The naming of the files must be web-safe (eg no characters such as / \ | # *) and preferably the file name should not include spaces. We ask that you keep your attachments under 2MB in total. If you have additional documents you should provide them when you are contacted by a QPSU Industrial Officer. Depending upon the number and the size of your attachments, it may take a while for the system to process your application. Please be patient and do not click the submit button more than once per screen.

Once you have completed the fom you will be presented with a date and time stamped copy of the information you have provided in the form. Please print and keep a copy of this preview page for your later reference.

Once you submit the completed form you will be forwarded to a web page that confirms the processing of the form. You will also receive a simple email that confirms processing. To protect your confidentiality this confirmation email contains very little information.
